On behalf of Divine Dharma Meditation Headquarters Center, I am very pleased to announce that a LEVEL 4 CLASS will be held in Kuala Lumpur, Malaysia, from August 21 to 23, 2017 (Monday to Wednesday).

Attached you will find the Class Enrollment Form. Please complete this Form fully and provide the following information for students from your Center:

1. Full name of student (last name, first name)
2. Gender of student (male or female)
3. Age of student
4. Date when student attended Level 3 Class (month, year)
5. Language for class lecture
6. Language for textbooks and CD
7. Hotel room (write "1" for all students in Room #1, write "2" for all students in Room #2, ...)

To facilitate efficient class arrangements, we ask that Center Leaders email the completed Enrollment Form back to us as soon as possible and no later than MARCH 31, 2017.
