Attached you will find the Class Enrollment Form. Please complete this Form fully and provide the following information for students from your Center:

1. Full name of student (last name, first name)
2. Gender of student (male or female)
3. Age of student
4. Date when student attended Level 3 Class (month, year)
5. Language for class lecture
6. Language for textbooks and CD
7. Hotel room (write "1" for all students in Room #1, write "2" for all students in Room #2, ...)
